Thesis Committee Report
This form needs to be completed for each thesis committee held. The student section should be filled out prior to the meeting and sent to the committee at least a day in advance. The form AND the student section are to be returned to the DSCB office immediately after the meeting. It is the student’s responsibility to collect the appropriate signatures and submit the form to the DSCB office.
